COVID-19 VACCINE IS AVAILABLE!!

Great news! – we have been given the information for you* to receive your vaccine, if you so choose. Please read this ENTIRE section and the links below carefully as they contain information if you have any trouble.

 

 HOW TO SCHEDULE AN APPOINTMENT FOR A VACCINE – This document gives you step by step instructions on how to sign up, and where to go.  This is a multi-step process!!  Make sure you complete the steps to the very end.

IMPORTANT POINTS:

  • Tangram is not requiring the vaccine at this time; however, we highly encourage it as the vaccines currently available in the US have been shown to be highly effective at preventing COVID-191.  More information is available here.

  • The link to schedule a vaccine is: https://scheduling.coronavirus.in.gov/vaccine (you can also click on it from the attached document).

  • This link SHOULD NOT be forwarded; it is currently only for Tangram employees who work with - or could potentially directly contact – a client.  The only exception is if someone was missed on the distribution (see below).

  • You must use Firefox or Chrome to access the link – NOT Internet Explorer or Edge.  We do not recommend using a cell phone at this time.

  • You are considered a healthcare worker for purposes of filling out this form.

  • You will be scheduled at a nearby hospital (near your home zip code).

  • For technical support, contact https://eportal.isdh.in.gov/c19VAXPublic/.  Tangram’s IT department/technical support will be limited to the information provided in these documents.

  • Once you receive your vaccine, please obtain a certificate that it is completed, and take a picture or scan to Stefanie Warren swarren@thetangramway.org in HR.

 

***Remember, this vaccine is a 2 shot process – make sure you schedule your second dose as soon as possible.***

Tangram's Response to COVID-19

Due to the fact that Tangram serves an aging, medically fragile population, it is critical that our employees take every precaution to limit exposure to COVID-19. Tangram is focused on protecting the health and safety of both our clients AND our employees. We have assembled an internal team dedicated to tracking the spread and impact of COVID-19 and ensuring Tangram’s preparedness and response to this new disease. Here are some of the steps we are taking:

  • Providing ample disinfectant, cleaning supplies, and personal protective equipment at client sites and office locations
  • Implementing quarantine procedures for clients and staff, as necessary
  • Training staff on the best ways to limit exposure to COVID-19
  • Providing up-to-date information and guidance from the Centers for Disease Control to employees
  • Increasing flexibility in our employee attendance policy to make it possible for employees with virus exposure or symptoms to stay home without penalty (see below for more information)
  • Maintaining open dialogue with local partners and health experts

COVID-19 HR Process (Updated 12/07/2020)
If you are experiencing any COVID symptoms or have been exposed to someone who has COVID
1. Stay home and quarantine.
2. Contact Stefanie Warren in HR (swarren@thetangramway.org) or by phone at (317) 968-9036 and your Program Manager to let them know if you are having symptoms, when you first started experiencing symptoms, if you have worked with any clients since experiencing symptoms or being exposed, and if you have taken a COVID test.
3. If you are missing any scheduled work because of COVID fill out the form at the following link: https://app.smartsheet.com/b/form/ec0f5ae67efa46139e3acc853e2e7403
4. HR will report any COVID positive employees to the State of Indiana.
 
Tangram Return to Work Criteria for DSP with Suspected or Confirmed COVID-19
DSP with mild to moderate illness who are not severely immunocompromised:
· At least 10 days have passed since symptoms first appeared and
· At least 24 hours have passed since last fever without the use of fever-reducing medications and
· Symptoms (e.g., cough, shortness of breath) have improved
Note: DSP who are not severely immunocompromised and were asymptomatic throughout their infection may return to work when at least 10 days have passed since the date of their first positive viral diagnostic test.
 
Test-Based Strategy for Determining when DSP Can Return to Work
DSP who IS symptomatic:
· Resolution of fever without the use of fever-reducing medications and
· Improvement in symptoms (e.g., cough, shortness of breath), and
· Results are negative and HR has received test results
DSP who IS NOT symptomatic:
· Results are negative and HR has received test results.

Dee Cooper
 
Dee Cooper is the Recruiting Coordinator and started in April 2020. She is a Ball State University Graduate as of July 2020 majoring in Human Resources Management and has a minor in Organizational Communications. Dee grew up in the small town of Lanesville Indiana right across the bridge from Louisville, Kentucky. Dee and her husband live in Muncie, Indiana with their dog named Rango and cat named Yangi. They spend their free time walking their pets, playing videogames, and being with family.
